1. This is the continued section from previous screenshot, which is the 'Form Fields Setting'.

2.There are several 'Form Fields' which you can select from such as 'Department', 'Priority', 'Product', and 'Order No'. These fields are optional depending on your business requirements.

3. Click to select multiple options to be displayed in your 'Add Ticket' form. Click on 'Save' button once you're done.
